Complete the following application, using the numbers for each section:

1. Principal Investigator Contact Information: include name, mailing address, phone number, and email address. 

2. Institutional Grants Administration Officer Contact Information.

3. Location where research will be conducted.

4. Project Title.

5. Lay abstract.  Provide a succinct lay summary of the project rationale and objectives.  (Specific aims will appear in the next section.)  200 word limit.

6. Specific Aims.  Please number them.

7. Will the research lead to a better understanding of headache or the treatment of headache patients? Describe how.  100 word limit.

8. Has work already begun on this line of inquiry in your lab? If yes, explain.  If no, can this research project be completed in one year?

9. Does this research duplicate other research, either in your lab or elsewhere?  Explain.

10. Do you have any pending applications for research funding that include this project?

11. Can you foresee any commercial value for the outcome of your research?  Explain.

12. Your Proposal.  Describe overall research plan.  Five page limit.
